OMS School Forms

Tonight, OMS Students should be coming home with the following paper copies of school forms:

• School Health Data Form
• Permission to Release Student Name/Picture Form
• Chromebook Acknowledgment Form
• Student Agreement for Use of Technology Form

Please sign and have your child return these forms to their first period teacher or the main office by Friday, August 28th.

In addition, we are piloting the use of online form signing, using Parent Square. With that said, please complete the following form online linked to this communication by Friday, August 28th:

Also, if you have updates to your address, phone, email, and/or emergency contacts, we ask you to please update that information online through the Power School Parent Portal. You may also contact the OMS main office to update this information as well by emailing front office staff Karen Dupuis kdupuis@oxps.org .  Please note that custodial parents are not permitted to remove another custodial parent without corresponding legal custody documentation.
